Maillot Jaune Miniature Poodle Application
ALL QUESTIONS MUST BE ANSWERED
Congratulations!

Applications (below) are registered and then secured by receipt of a non-refundable
$500 deposit.  Registration is done on a first-come first-serve basis.  We will contact
you after we have received your on-line application (below). If you have not heard from
us within 24 hours please e-mail info@bodyandmindsolutions.com. Should the
deposit not be received within 4 days of the application, your priority pick will be
forfeited. Your deposit can be paid via:
  1. PayPal (You can use your credit card to pay with Pal account. Payments are
    secured and guaranteed by PayPal.  If you choose to pay via PayPal, you will
    need to include the cost plus an additional 3% to cover PayPal commissions.
    For instance, an additional 3% on a $500 deposit would be $15, thus bringing
    the deposit total to $515.00.

We will contact you with as soon as the puppies are born and pictures will be posted
online and/or e-mailed to you. We will work with you to match you with a puppy that
best suits your needs and work with you on any travel arrangements that need to be
made to welcome your puppy home. Also, at this time, your final payment will need to
be made by PayPal or direct deposit. You will also need to print, sign, and send 2
copies of the Health Warranty Contract (found at the bottom of the Resources page).  

Buyers placing deposits on current litters, agree that the puppy will be paid for by week
4, and picked up/shipped by 8 weeks of age. One (1) week grace period is allowed by
either party; however, after the grace period, a daily boarding fee of $10.00 per day will
be charged as well as any veterinarian fees that occur during such boarding time.  
Puppy will be held for a maximum of 2 weeks under this condition unless other
arrangements are agreed upon. Otherwise, the deposit will be forfeited and the puppy
will be placed back on "Puppies Available."

We will be available for ongoing support for as long as you need! Please feel free to
